To Order Custom Labels

• Select your wine from the MadoroM Store, add the desired number of custom labels (in increments of six) & complete your purchase
• Return to the Custom Labels page to fill out the form below and attach your artwork
• In the "Label text and information" box, please include your order number & any text you'd like on the label
• If you would like Hunter or Angler labels, please include the details of your harvest including the Hunter/Angler's name, animal type, size if applicable, date and location of your hunt
• For wedding labels, please include the couples married names, date and location of wedding
• A label proof will be emailed to you for your approval prior to printing

        *If you would like to submit multiple custom labels, please fill out a separate form for each piece of artwork.
